FROM THE NORTHERN STATE:
Exit 26S, New Hyde Park Rd. Make a right at second blue & gray “Lake Success Quadrangle” sign. Head straight into complex and make a left onto Vermont Dr. (3rd set of buildings on left). Tip: Our awning says 5 PROHEALTH, which is our building number (but our address is 3 VERMONT DR.)
LONG ISLAND EXPRESSWAY - West Bound:
Exit 34 to New Hyde Park Rd. Make Left onto New Hyde Park Rd. Make a right at second blue & gray “Lake Success Quadrangle” sign. Directions
From the Long Island Expressway:
Take Exit 49N (Route 110 - Huntington).
Follow Route 110 north until it becomes New York Avenue.
Turn left onto West Carver Street.
Office building is on the left.
From the Northern State Parkway:
Take Exit 40N (Route 110 - Huntington).
Follow Route 110 north for approximately 5.5 miles.
Turn left onto West Carver Street.
Office building is on the left.
Alternate Route from Long Island Expressway or Northern State
Parkway:
Take Exit 51N (Deer Park Avenue) off of the Long Island Expressway
or Exit 42N (Deer Park Avenue) off of the Northern State Parkway.
Follow Deer Park Avenue north over Route 25 (Jericho Turnpike).
Turn left onto Route 25A (Northern Boulevard/Main Street) toward
Huntington Village.
After second traffic light, turn left onto Prospect Avenue.
Turn right onto West Carver Street.
Office building is on the left.
From Northern Boulevard (Route 25A) East-Bound:
Continue past Cold Spring Harbor.
After entering Huntington Village, make a right onto Prospect Avenue.
Turn right onto West Carver Street.
Office building is on the left.
From Northern Boulevard (Route 25A) West-Bound:
After entering Huntington Village, continue past Route 110.
After second traffic light, turn left onto Prospect Avenue.
Turn right onto West Carver Street.
